On Tuesday Oct 9th, all FPSA students will be given an important information package from Ottawa Public Health regarding their upcoming immunization clinic at FPSA (Oct 15,16,17). Please see below for additional instructions for parents.

Please note: As they pertain to confidential medical information regarding your child, school staff are not able to provide additional information about your child's immunizations, or alternative options for dealing with immunizations. How does my child get the vaccine?

All students in grade 7, (students born in 2006), will receive a package at school with consent forms for hepatitis B, human papillomavirus (HPV) and meningococcal disease vaccines  one (1) to two (2) weeks before the clinic date.  If you don’t see a package speak with your child.

These vaccines are publicly-funded, so no payment is needed.

What do I do with the package?

Both you and your child should:

  1. Read the vaccine fact sheets 2) Sign the consent form for EACH vaccine you want your child to get.

On the day of the school clinic, your child should:


  • Eat a good breakfast.
  • Bring an extra juice or snack to school.
  • Wear short sleeves or a loose shirt so that the upper arm can be exposed easily, without undressing.
  • Bring the consent form to school.

Please note for safety, confidentiality and efficiency, we are not able to accommodate parents who wish to remain with their child during immunization in our school clinics. To discuss alternatives such as our daily Constellation clinic and evening catch-up clinics please call  613-580-6744.

Grade 7’s who miss the Fall clinic will be able to receive their vaccines at the Spring clinic. If your grade 8 child missed a vaccine in grade 7, or if you have questions about the grade 7 vaccines please call Ottawa Public Health (OPHIC) at 613-580-6744 and ask to speak to a nurse.

Santé publique Ottawa organisera des cliniques de vaccination à l'école de votre enfant___________

Ces vaccins financés par l'État sont gratuits.


Tous les élèves de 7e année et les élèves nés en 2005 recevront à l'école les formulaires de consentement pour l'obtention des vaccins contre l'hépatite B, le virus du papillome humain (VPH) et la méningococcie.


Les élèves sont encouragés à parler des vaccins avec vous; idéalement, vous et votre enfant devriez signer le formulaire de consentement. La journée de la clinique, n’oubliez pas de :


  • Prendre un bon déjeuner.
  • Apporter un jus ou une collation de surplus.
  • Porter des vêtements avec des manches courtes ou amples, pour faciliter l’accès au bras sans se dévêtir.
  • Apporter les formulaires de consentement eux-mêmes à la clinique d'immunisation en milieu scolaire.


Les élèves absents à une clinique pourront recevoir le vaccin lors de la clinique d'immunisation en milieu scolaire suivante.
